Service Description:
Psychosocial Rehabilitation Services provide individuals with an opportunity to work in groups with mental health providers in development of goals and objectives for the following trainings and reinforcements:
- educate individuals with mental, behavioral, or emotional illness about mental illness
- educate individuals about appropriate treatment to avoid complications and relapse.
- provide opportunities to learn and use independent living skills, and
- enhance social and interpersonal skills within a supportive and normalizing program structure and environment
Areas of focus for the program include: Rehabilitative Interventions, Interpersonal Skills, Daily Living Skills, and Independent Living Skills

Eligibility:
1. Individuals must have a current or recent mental health occurrence resulting in significant functional impairments in major life activities.
2. Additionally, the individual must meet at least two of the following criteria:
- Difficulty in establishing or maintaining normal interpersonal relationships that puts them at risk of hospitalization, homelessness, or isolation from social supports.
- Exhibits inappropriate behavior that requires repeated interventions by the mental health, social services, or judicial systems.
- Difficulty in recognizing personal danger or recognizing significantly inappropriate social behavior.
- Require help in basic living skills that puts their health or safety in jeopardy.
3. Additionally, the individual must meet at least one of the following criteria:
- Have experienced long-term or repeated psychiatric hospitalizations; or
- Lack daily living skills and interpersonal skills; or
- Have a limited or non-existent support system; or
- Be unable to function in the community without intensive intervention; or
- Require long-term services to be maintained in the community.
