Service Descriptions:
  
High Intensity Outpatient Program (HIOP) ASAM 2.5
  - This program lasts for 2-4 weeks based on the individual’s needs. The individual participates in therapeutic groups 5 days a week - 4 hours each group. An array of topics are tailored to the groups.
Intensive Outpatient Program (IOP) ASAM 2.1
  - This program lasts between 8-10 weeks based off the individual’s needs. It consists of 3 days a week – 3 hours each group. It allows individuals to maintain their daily routines and responsibilities, while receiving person-centered substance abuse treatment.
Individual Counseling & Dual Diagnosis ASAM 1.0
  - Individual counseling and groups, as needed, are offered for maintenance and ongoing supports. Additionally, counseling sessions for individuals with Dual Diagnosis of substance abuse and mental health diagnosis are available.
Coordinated Medicated Assisted Treatment (MAT)
  - Medication-assisted treatment (MAT) is an evidenced based, whole person approach, utilizing medication in combination with counseling, behavioral therapies, and peer support for the treatment of substance use disorders. For those with an opioid use disorder (OUD), medication addresses the physical difficulties a person may experience when they stop taking opioids. MAT can help to re-establish normal brain function, reduce substance cravings, and prevent relapse.
Eligibility: 
Individuals are assessment based on the ASAM Six Dimensions Assessment to determine the Level of Care most appropriate (e.g., ASAM 2.5, ASAM 2.1, ASAM 1.0). Individuals who are determined to need a higher level of care or different services will be assisted with Care Coordination and referrals to appropriate services. 
Individuals with Medicaid and Medicare are eligible. Call for information on accepted MCO plans.
Program Hours:  9 am to 4 pm and by scheduled group sessions.
